debian: add debian/source/format

From Thomas Hood:
The file that should be present is debian/source/format, containing
the following line "3.0 (quilt)".  This causes dpkg-source to build source
files in the new, recommended, source format.  For details see, e.g.,
http://wiki.debian.org/Projects/DebSrc3.0
